In addition to the Background Check, documentation of the following must be submitted by week six of the program.

• Current CPR card:
  • Must be Basic Life Support for Healthcare Providers through the American Heart Association
  • 303.980.1015 or www.americanheart.org
• Immunizations (Vaccines or Titers are acceptable)
  • Hepatitis B Vaccine:
    • Two of the three injections are required for clinical placement
    • NOTE: There is a one-month wait between injection one and two
  • Two Measles, Mumps, Rubella vaccines
  • Chicken Pox vaccine, titer or documentation of disease by a healthcare professional
  • A negative TB test or negative chest X-ray report
    • TB testing is required annually for all healthcare providers
  • Tetanus within the last 10 years
  • Seasonal Flu Shot
